Abstract The aim of this study is to provide a real picture of the disease burden of Prameha in society. The study was performed in Government Ayurved College and Hospital, Nagpur, Maharashtra during Oct 2015-Mar 2016. Total 60 patients of newly diagnosed type 2 diabetes mellitus attending the Kayachikitsa Opd of GAC Nagpur were included for the study. The subjects details were recorded in case report form. The CRF included many variables such as sociodemographic factors, presenting symptoms, risk factors such as hypertension, obesity and glycaemic status, family history of diabetes and physical activity. Other parameters like BMI, glycosylated haemoglobin, fasting and post prandial blood sugar and fasting lipid profile were documented. Descriptive and bivariate analyses were carried out using the XLSTAT software (2020). Amongst 60 subjects, 65% were male and 93.3% were adults. 78% of subjects were following sedentary lifestyle and 40% had family history of diabetes. The results revealed that, obesity, family history of diabetes, uncontrolled glycemic status, sedentary lifestyles, and hypertension were prevalent among the Prameha subjects. The characterization of this risk profile and early detection of prameha by observing poorvarupa will contribute to designing more effective and specific strategies for screening and controlling Prameha in Maharashtra, India.

ABSTRACT Purpose: To describe microvascular changes in the maculas of individuals with type 2 diabetes observed on optical coherence tomography angiography (OCTA) images. We compared the maculas of diabetic subjects without diabetic retinopathy with those of healthy subjects and correlated the findings with the clinical profiles of diabetic subjects. Methods: One eye each of 30 patients with diabetes and 30 healthy individuals were examined. The patients with diabetes underwent funduscopy, retinography, and fluorescein angiography to rule out retinopathy. All subjects underwent optical coherence tomography angiography of a macular area (6×6 mm2), and the foveal and parafoveal vascular densities were analyzed in the superficial and deep retinal vascular plexus. The foveal and parafoveal thicknesses, foveal avascular zone of the superficial plexus, and choriocapillaris flow area were also examined. The optical coherence tomography angiography results were compared between the two study groups and correlated with the following parameters: visual acuity, time since diabetes diagnosis, glycemic control, lipid profile, and renal function of patients with diabetes. Results: A minimal increase in the choriocapillaris flow area was observed in the patients with diabetes (mean area, 22.3 ± 4.6 mm2 in controls; 22.6 ± 3.9 mm2 in patients with diabetes) (p=0.017). No significant differences were observed between other optical coherence tomography angiography parameters analyzed in the two groups. Glycosylated hemoglobin and fasting blood glucose levels were significantly negatively correlated with the foveal vascular density of both plexuses; conversely, fasting blood glucose levels were positively correlated with the choriocapillaris flow area (p=0.034). The other clinical parameters were not correlated with the optical coherence tomography angiography findings. Conclusion: Optical coherence tomography angiography may not be the most appropriate tool for detecting preclinical changes in patients with diabetes, moreover, optical coherence tomography angiography; does not replace clinical examinations. Glycemic control should be the primary clinical parameter considered during retinopathy screening. Larger studies are necessary to confirm these findings.


RESUMO Objetivo: Descrever alterações microvasculares na mácula em diabéticos do tipo 2 sem retinopatia diabética e pacientes saudáveis, e correlacionar achados com perfil clínico nos diabéticos. Métodos: Foram incluídos 60 olhos de 30 diabéticos e 30 pacientes saudáveis. Diabéticos realizaram fundoscopia, retinografia® (CR2; Canon Inc., New York, New York, USA) e angiografia fluoresceínica® (TRC-50DXC; Topcon Inc., Tokyo, Japan) para descartar a presença de retinopatia. Os 60 pacientes realizaram a angiografia por tomografia de coerência óptica® (RTVue XR, Avanti, Optovue, Fremont, CA, USA) (área macular: 6 x 6 mm2) e foram analisados densidade vascular total, foveal e parafoveal no plexo capilar superficial e plexo capilar profundo, espessura foveal, espessura parafoveal, área da zona avascular da fóvea no plexo capilar superficial e área de fluxo da coriocapilar. Resultados da angiografia por tomografia de coerência óptica foram comparados entre os 2 grupos e correlacionados com acuidade visual, tempo de diabetes, controle glicêmico, perfil lipídico e função renal nos diabéticos. Resultados: Observou-se aumento mínimo da área de fluxo da coriocapilar nos diabéticos, média das áreas foi de 22,3 ± 4,6 mm2 no grupo controle e 22,6 ± 3,9 mm2 em diabéticos (p=0,017). Não foi observada diferença estatisticamente significante entre outras variáveis da angiografia por tomografia de coerência óptica analisadas nos dois grupos. Hemoglobina glicosilada e glicemia de jejum apresentaram correlação negativa estatisticamente significante com densidade vascular foveal de ambos os plexos e a glicemia de jejum se correlacionou positivamente com área de fluxo da coriocapilar (p=0,034). Outros dados clínicos avaliados não apresentaram correlação com achados da angiografia por tomografia de coerência óptica. Conclusão: Resultados sugerem que a angiografia por tomografia de coerência óptica pode não ser a melhor ferramenta na detecção de alterações pré-clínicas em diabéticos, não substituindo o exame clínico, e corroboram a ideia de que o controle glicêmico deve ser o principal parâmetro clínico a ser considerado na triagem da retinopatia. Estudos com amostras maiores são necessários para confirmar os achados.

Introducción: La esteatosis hepática no alcohólica es la enfermedad con mayor frecuencia en el mundo, esta se asocia a diabetes mellitus tipo 2; padecimiento con gran impacto económico-social. Objetivo: Describir el comportamiento de esteatosis hepática en una población de pacientes con diagnóstico de diabetes mellitus tipo 2. Métodos: Se realizó un estudio observacional, descriptivo, transversal en 94 pacientes diabéticos atendidos en el Hospital Clínico Quirúrgico Hermanos Ameijeiras, en el periodo comprendido entre febrero 2016- febrero 2018. Resultados: La esteatosis fue leve en 42,6 por ciento, moderada (44,7 por ciento) y severa en 12,8 por ciento, en ambos casos, con un NAFLD score principalmente indeterminado (72,3 por ciento). Se detectó fibrosis F0-1 en 58,5 por ciento de los pacientes, F2 (29,8 por ciento) y significativa (F3 y F4) en 11,7 por ciento). Se detectó asociación entre ecogenicidad hepática aumentada, patrón hepático difuso y grado severo de esteatosis por ecografía con mayor grado de fibrosis según elastografía. Se detectó relación significativa de niveles elevados de HBA1C con fibrosis significativa medida tanto por NAFLD score como por elastografía. Conclusiones: La esteatosis hepática presente en los pacientes con diabetes mellitus fue principalmente leve a moderada con prevalencia de fibrosis leve, el grado de fibrosis significativa se asoció con ecogenicidad hepática aumentada, patrón hepático difuso, grado severo de esteatosis por ecografía y niveles elevados de hemoglobina glucosilada(AU)


Introduction: Non-alcoholic liver steatosis is the most frequent disease in the world. It is associated with type 2 diabetes mellitus, representing great economic-social impact. Objective: To describe the behavior of hepatic steatosis in a population of patients diagnosed with type 2 diabetes mellitus. Methods: An observational, descriptive, cross-sectional study was conducted in 94 diabetic patients treated at Hermanos Ameijeiras Clinical Surgical Hospital, from February 2016 to February 2018. Results: Steatosis was mild in 42.6 percent, moderate (44.7 percent) and severe in 12.8 percent, with a mainly indeterminate NAFLD score (72.3 percent). F0-1 fibrosis was detected in 58.5 percent of the patients, F2 (29.8 percent) and significant (F3 and F4) in 11.7 percent). Association between increased liver echogenicity, diffuse liver pattern and severe degree of steatosis were ostensible by ultrasound with higher degree of fibrosis according to elastography. Significant relationship of elevated levels of HBA1C with significant fibrosis was detected, measured both by NAFLD score and by elastography. Conclusions: Hepatic steatosis present in patients with diabetes mellitus was mainly mild to moderate with prevalence of mild fibrosis, the degree of significant fibrosis was associated with increased liver echogenicity, diffuse liver pattern, severe degree of steatosis by ultrasound and high levels of hemoglobin. glycosylated(AU)

OBJECTIVES: Nonalcoholic fatty pancreas disease (NAFPD) is characterized by excessive fat deposition in the pancreas in the absence of alcohol consumption. In this study, we aimed to detect a possible relationship between adipose tissue accumulation, prediabetes and diabetes. METHODS: This cross-sectional and retrospective study included 110 patients. Three groups were classified as controls, patients with prediabetes and patients with type 2 diabetes. The abdominal computed tomography (CT) attenuation measurement results of the pancreas were evaluated independently by two experienced radiologists. CT measurements and biochemical parameters were compared between study groups. The relationship between continuous variables was assessed by using one-way ANOVA. To determine the changes in the dependent variable for the effects on study groups, the independent variable was adjusted using ANCOVA. A p-value less than 0.05 was considered statistically significant. RESULTS: The presence of prediabetes and type 2 diabetes was correlated with a decrease in the mean Hounsfield Unit (HU) value of the pancreas (p=0.002). Age was determined to be an independent risk factor and was correlated with NAFPD (p=0.0001). When compared to the controls (p=0.041), 71% of patients with prediabetes and 67% of patients with type 2 diabetes were observed to have an increased incidence of NAFPD. Decreased serum amylase was found to be correlated with the mean HU value of the pancreas (p=0.043). CONCLUSION: NAFPD was independently correlated with both prediabetes and type 2 diabetes adjusted for age (p=0.0001) in this study. Additionally, age was determined to be an independent risk factor and was correlated with NAFPD.

AIMS AND OBJECTIVE: Ultrasonographic determination of gallbladder volume in diabetics [both type I and type 2], it's comparison with a control group, and correlation of gallbladder volume in diabetics with parameters such as age, sex, body mass index, parity, hyperlipidaemia, and autonomic neuropathy. MATERIALS AND METHOD: Ninety one cases of diabetes mellitus and 40 healthy controls were recruited for the study. A detailed history and physical examination were recorded. Laboratory investigations done were--fasting and postprandial blood sugar, glycosylated haemoglobin, microalbuminuria, and serum lipid profile. Autonomic neuropathy was determined by using simple non-invasive bedside tests. Fasting gallbladder volume was measured by ultrasonography (calculated by ellipsoid formula). RESULTS: The mean fasting gallbladder volume was 18.20 +/- 2.54 ml in type I diabetics and 25.87 +/- 13.90 ml in type 2 diabetics, with a minimum value of 9.30 ml and maximum value of 88 ml. When type 2 diabetics were subgrouped according to the presence of autonomic neuropathy, higher gallbladder volumes were seen in patients with autonomic neuropathy. CONCLUSIONS: Cholecystomegaly, to a significant degree, was found in type 2 diabetics in the present study. It was significantly correlated with age, body mass index, and the severity of autonomic neuropathy. In male type 2 diabetics, gallbladder volume was significantly correlated with LDL cholesterol levels. In female type 2 diabetics, gallbladder volume was significantly correlated with waist-hip ratio. Gallbladder volume also had significant correlation with proliferative diabetic retinopathy, but not with glycaemic control, microalbuminuria, hypertension, or the duration of diabetes.
